*** HAZARD SUMMARY:  ***  CRITICAL INJURY - COPIER MACHINE  
  
  A worker was critically injured after failing to lock out a copier  
  machine while performing maintenance work.  After removing the guards,  
  the worker reached in to clean the chain and accidentally pressed the  
  power button.  This started the press which drew in her arm and  
  resulted in a fracture and multiple abrasions/cuts.

  Photocopying and duplicating machines.  
  
  
*** APPLICABLE SECTION(S) OF INDUSTRIAL REGULATIONS ***  
  
  This critical injury could have been prevented if appropriate lock  
  out procedures had been in place and followed.  
  
  Section 46 of the Regulations for Industrial Establishments states that:  
  
        "... the power supply to electrical installations, equipment or  
        power lines shall be disconnected and locked out of service prior  
        to any work being done on, or in proximity to, the installations,  
        equipment or power lines."  
  
  Section 79 of the Regulations for Industrial Establishments states that:  
  
        "A part of a machine, transmission machinery, device or thing  
        shall be cleaned, oiled, adjusted, repaired or have maintenance  
        work performed on it only when,  
  
        (a)  motion that may endanger a worker has stoppped;  and  
        (b)  any part that has been stopped and that may subsequently  
             move and endanger a worker has been blocked to prevent its  
             movement."  
  
  Section 80 of the Regulations for Industrial Establishments states that:  
  
        "Where the starting of a machine, transmission machinery,  
        device or thing may endanger the safety of a worker,  
  
        (a)  control switches or other control mechanisms shall be  
             locked out;  and  
        (b)  other effective precautions necessary to prevent such  
             starting shall be taken."
