He fell approximately 15 feet to the ground where he was found unconscious with the ladder on top of him. He died later in the hospital. *** APPLICABLE SECTION(S) OF INDUSTRIAL REGULATIONS *** 1. All workers to be trained in the recognition of potential hazards associated with their work. 2. Where possible the top of the ladder shall be securely fastened. Part 16(7), Reg. 77-1. 3.
