The tanks on the truck along with their related piping had not been purged and tested properly before the cutting torch was used. *** APPLICABLE SECTION(S) OF INDUSTRIAL REGULATIONS *** 1. All workers are to be trained in the recognition of potential hazards associated with their work. 2. Hot work should only be conducted on containers which may have held flammable or explosive substances when a proper written procedure is followed. 3.
