*** HAZARD SUMMARY:  WORKER KILLED IN EXPLOSION ***  
  
  A worker was killed as a result of an explosion which occurred while he  
  was using a welding torch to dismantle a fuel delivery truck.  
  
  The tanks on the truck along with their related piping had not been  
  purged and tested properly before the cutting torch was used.  
  
  
*** APPLICABLE SECTION(S) OF INDUSTRIAL REGULATIONS ***  
  
  1.  All workers are to be trained in the recognition of potential  
      hazards associated with their work.  
  
  2.  Hot work should only be conducted on containers which may have  
      held flammable or explosive substances when a proper written  
      procedure is followed.  
  
  3.  Employers and workers are to adhere to the requirements of  
      regulation 77-1 (Specifically subsections 3(7) and 27(6)) under  
      the Occupational Health and Safety Act.
