The incident occured when he noticed a conveyor chain had jammed. To free the chain, he crossed on a second moving conveyor chain and got caught, causing his leg and hip to be crushed against an idler (support) roller. *** APPLICABLE SECTION(S) OF INDUSTRIAL REGULATIONS *** 1. All employees are to be trained in the recognition and avoidance of hazards associated with their work. 2.
