Worn or damaged equipment may cause injury or electrocution. Some types of industrial plugs and receptacles rely on a raised rib or keyway in the outer casing (see diagram) for correct polarity. If the rib is worn or damaged, the ground terminal of the plug cap can be mated with an energized phase terminal of a receptacle. Proper maintenance of the equipment, and replacement of plugs and receptacles when worn or damaged, can prevent a hazardous situation. All receptacles and plug caps should be examined before using. Any damaged or worn units should be replaced. All plugs must conform to 'The Canada Electrical Code', Part I.
