The power had been shut off. An anti-roll-back device was in place which created unbalanced tension in the belt. This energy, or unbalanced tension, caused the pulley to rotate when the worker released the jam, drawing him between the belt and the pulley. *** LOCATION(S) AND SECTOR(S) WHERE HAZARD MAY BE ENCOUNTERED *** *** APPLICABLE SECTION(S) OF INDUSTRIAL REGULATIONS *** 1. All workers are to be trained to recognize and avoid the hazards associated with their work. (Paragraph 9(2)(b), Occupational Health and Safety Act) 2. Workers are never to position themselves where they may come into contact with equipment that is moving or has the potential to move. (Part 26(4), Regulations 77-1) 3. Workers are not to clean or repair a conveyor unless it is locked out.
