The victim was attempting to climb the ladder on the side of a moving harvester when she slipped and fell into the path of the harvester wheel. This harvester had a straight ladder with no platform at the bottom. Anyone mounting or dismounting the machine had to stand directly in front of the wheel. *** PROTECTIVE MEASURES *** 1. That a ladder with a platform at the bottom and hand rails on both sides by installed on the side or rear of the harvester. 2. That no one be allowed to mount or dismount moving machinery. 3. That a warning decal be visibly placed to indicate the dangers of mounting or dismounting a moving machine. 4. That a signal device be installed on the harvester so the crew can alert the tractor driver in any emergency situation. 5. That the employer provide such information, instruction, training, and supervision as is necessary to ensure an employee's health and safety.
