*** HAZARD SUMMARY:  IMPROPER INSTALLATION OF TROLLEY TRACK HOIST ***  
  
  A potential hazard exists when a roofer's power driven trolley track  
  hoist is used when improperly installed with the stay wire and spring  
  lock pins in place.  
  
  A worker lost his life when a hoist topppled over causing him to fall  
  over the of the building.  The hoist did not have the stay wire in  
  place.  
  
  
*** PROTECTIVE MEASURES ***  
  
  1.  All workers to be trained in the recognition of potential hazards  
      associated with their work.  
  
  2.  A hoist should not be used unless it is installed in accordance  
      with the manufactureers' specifications.
