* * * * * * * * * * * * * * * * * * * * * * * * * * * * * * * * * * * * * * INFORMATION ALERT * * * * Produced by: Ontario Ministry of Labour * * Provided by: Canadian Centre for Occupational Health and Safety * * * * * * * * * * * * * * * * * * * * * * * * * * * * * * * * * * * * * * INFO ALERT: 9 DATE: March 4, 1991 *** HAZARD SUMMARY: *** CRITICAL INJURY - INJECTION MOULDING In two separate incidents, two workers received serious crushing injuries to and amputations of their hands when they attempted to reach under the guards on injection moulding machines, to make adjustments to the moulded parts without first locking out the equipment. *** LOCATION(S) AND SECTOR(S) WHERE HAZARD MAY BE ENCOUNTERED *** Injection moulding machines. *** APPLICABLE SECTION(S) OF INDUSTRIAL REGULATIONS *** These injuries could have been prevented if the machines were properly guarded. Section 28 of the Regulations for Industrial Establishments states that: "Where a machine or a prime mover or transmission equipment has an exposed moving part that may endanger the safety of any worker, the machine or prime mover or transmission equipment shall be equipped with and guarded by a guard or other device that prevents access to the moving part." Section 46 of the Regulations for Industrial Establishments states that: "(1) Subject to subsections (2) and (3), the power supply to electrical installations, equipment or power lines shall be disconnected and locked out of service prior to any work being done on, or in proximity to, the installations, equipment or power lines." Section 79 of the Regulations for Industrial Establishments states that: "A part of a machine, transmission machinery, device or thing shall be cleaned, oiled, adjusted, repaired or have maintenance work performed on it only when, (a) motion that may endanger a worker has stopped; and (b) any part that has been stopped and that may subsequently move and endanger a worker has been blocked to prevent its movement." * * * * * * * * * * * * * * * * * * * * * * * * NOTE: "Information Alert" is produced by the Industrial Health and Safety Branch, Ministry of Labour.
